Here is my story and
how your generosity and gift can help.
I am a Social Worker/Case Manager. I assist
people with physical and developmental disabilities
find services to remain in their own homes and
communities. My company is called Choice &
Options, Inc. and is mainly funded by our State
Medicaid Waiver program, however, recent cuts in the
Medicaid funding has forced my company to struggle
with staying in business! We have 20 Case
Managers that assist over 200 clients, however recent
budget cuts and changes in regulations have increased
our work but reduced our rate of pay! Without a
Case Manager, our clients will be unable to find the
resources they need to stay as independent as possible
and to remain living in their own home and own
community. |

Recent changes in tax
requirements and certification requirements have
caused my company to struggle with meeting payroll tax
requirements, meeting payroll, providing assistance to
persons who are in need of adaptive equipment, and as
a result, has caused us to resort to attempting to
find help and funding elsewhere! I am unable to
get a business loan, because of the nature of business
we do. We have very little collateral because we
work out of our own homes. I also owe payroll
taxes and because of owing taxes, the banks are
unwilling to grant us a loan. |

I am desperately in
need of financial assistance and am willing to make
arrangements to pay a loan back, offer our expertise
in case management in exchange for a donation or loan,
or donate my services to an organization that may be
of particular interest to you. My goal is
to raise $65,000.00 however, I would be grateful if I
only was able to raise $1000.00! Please help my
company and my clients out and make a donation today! |
